Job summary

REACT is a high intensity home treatment team which works very closely with all teams in MHSOP Community, particularly Day Hospital Services. The team works with people 65+ (and people with Younger Onset Dementia) in mental health crisis and also with patients who require more intense support on discharge from hospital. Working collaboratively and fostering positive relationships with community and hospital teams is key to the role.

The post holder will be managerially and professionally responsible for the community mental health nurses and support workers based within REACT. They will work alongside the Clinical Lead Nurse who is responsible for the clinical oversight of the caseload and development and training of staff. They will be supported within the role by the Senior Nurse and will also have a close working relationship with the Consultant Psychiatrist.

The REACT manager will ensure a high standard of care and treatment is delivered for service users in line with the Mental Health Measure (2010). This will be facilitated by regular supportive supervision of staff. The Manager will also play a key role in the strategic development of services as per the goals of the MHSOP directorate.

Main duties of the job

Mental Health Services for Older People are looking for a dynamic registered mental health nurse, with experience in community facing services, who can managerially lead a team providing mental health assessment and high intensity care and treatment for older people. REACT is an evolving service and you will be expected to facilitate quality improvements and patient focused outcomes.

The REACT Team is a multi-disciplinary team based in MHSOP Community Services in Llanfair Unit, University Hospital Llandough (UHL). The team provides emergency assessments and enhanced high intensity interventions for older people with severe and unstable mental health. REACT's remit is to provide an alternative to hospital admission for people who are best supported at home, promote appropriate access to MHSOP inpatient beds and to support people with timely discharge back into their community settings. There is an emphasis on mental health assessment, risk assessment and management, person centred care, psychotherapeutic interventions and collaboration/ joint working with other mental health services within MHSOP.
