Job summary

You will be responsible for triaging referrals and carrying out home visits to prevent further decline or admission to hospital by providing urgent care, equipment and specialist older person's multidisciplinary assessment. The service will be developing into the Safe at Home team, which will aim to prevent hospital admission by managing acutely deteriorating patients in the community.

Main duties of the job

An exciting opportunity to join the newly created Safe@Home Service within Cardiff and Vale University Health Board. We are recruiting to a Band 6 Clinical Nurse Specialist post and offer this to clinical staff with good IT knowledge and skills, that are keen to work in a busy, friendly and developing service. Ideally we would like you to have community/primary care experience.

The right candidate for this post will need to display excellent communication skills, problem solving and documentation with attention to detail and the ability to manage challenging and changing situations. They will require experience with managing deteriorating patients and have good clinical experience. They will be expected to work flexibly across the Cardiff and Vale bases and be available for home visits. They will also need to have a willingness to learn and be involved in service developments.

The post will involve working in partnership with the Cardiff Community Resource Team and collaboratively with related specialities across the UHB.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
