Job summary
An exciting opportunity has arisen for a Nutrition Nurse Specialist within our Community Nutrition Support Team. The successful candidate will be based at Riverside Health Centre, and will be required to provide care to patients in a variety of settings across the Cardiff and Vale health board geographical area, including patient's homes. The ability to work as part of a team is essential to this role.
Interested candidates must have excellent communication skills, enjoy MDT working and have experience of enteral nutrition.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The main duties of the post include: visits to patients in a variety of settings, including patient's homes, in order to: assess, plan, deliver and evaluate patient care using a holistic and individualised approach; liaising with family members, carers, community nurses and other HCPs to implement care plans for the management of enteral feeding devices and stoma sites; training and education relating to care of stoma sites and enteral feeding tubes; effective communication with wider MDT.
