Job summary
An exciting opportunity had arisen for an enthusiastic and dynamic pharmacist to join the Primary, Community and Intermediate Care Medicines Management Team, within the Clinical and Analytics workstream.
Following a reorganisation of the team the post holder will be supporting the workstream lead to ensure programmes of work are developed and carried out effectively in relation to medicines optimisation. With your knowledge of national and local guidance and advice, you will respond to specific queries regarding medicines optimisation and also help identify and develop plans to implement cost effective and evidence based prescribing within primary care, and the wider Health Board. You will be expected to provide high quality evidence based prescribing advice and develop good professional relationships with a range of medical and non-medical prescribers across primary care.
You will have excellent communication skills and have the ability to work independently as well as part of a team. You will be expected to engage with other pharmacy colleagues and health and social care professionals across all interfaces to ensure a seamless approach to medicine management.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply,
Main duties of the job
- To improve the safe, rational and cost-effective prescribing to a locality group of GP practices and members of the Primary Healthcare Team.
- To support practices in the implementation of approved evidence-based prescribing strategies through implementation of the LHB prescribing strategy.
- To develop the primary care pharmacist role in GP practices delivering services to patients.
- To develop the work programme of the Analytics / Medicines Optimisation workstream.
