Job summary
We have an exciting opportunity to be part of the continuing development of our services to improve the support we provide to Mental Health clinical teams in discharge planning. Also to provide education and training in relation to discharge planning for health and social care professionals.
Do you enjoy working as part of a multi-disciplinary team, have an interest in services that span both acute hospital and community services and enjoy working collaboratively with patients to identify the support required to meet their ongoing needs following a hospital stay?
Would you like to be able to influence the development of ward team's expertise in discharge planning and be involved in strategic plans to improve the management of patient flow through hospital services?
Main duties of the job
We are looking for someone to join our innovative and dynamic Integrated Discharge Service. This team comprises of Nurses, Social Workers, OT, Administrative staff as well as a range of 3rd sector workers, working with us to support discharges from hospital. The key aim of this team is to provide expert support to ward staff across all hospital specialties to identify and coordinate complex discharge plans for their patients. Assessment, care planning, negotiation and problem solving skills are essential to this role.
You will be working mainlywith our Older Adult Mental Health Units based at University Hospital Llandough with input to Young Onset Dementia Services ,Neuropsychiatry and Adult Mental Health as required. Management and support will be via the Mental Health Clinical Board Complex Care and Commissioning Team with your base at Woodland House.
If you are resilient and can think creatively, communicate effectively and are motivated and want to work as part of our small team, if you are excellent at communicating and resolving conflict and have negotiating skills then this may be the post for you.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
