Job summary
The health inclusion HCSW works as part of the CAV Health Inclusion Service to improve the health of the most vulnerable patient groups in the Cardiff & Vale area. The health inclusion HCSW will take a positive, innovative and solution focused approach to providing timely care for people experiencing poor health outcomes and varying health complexities. The role will be rotational in nature across the scope of CAVHIS, this will include working within our central hubs in the Cardiff Royal Infirmary and the emergency department of the UHW, but also within an outreach capacity in venues such as homeless hostels, probation premises, sex parlours, street outreach, refugee and asylum seeker settlements and as part of the council run homeless MDT. Flexibility to work within these areas and the wider CAVHIS team is paramount.
Main duties of the job
The health inclusion HCSW will work closely with nursing colleagues and professionals from other specialities and alongside tertiary agencies, to encompass the best achievable health outcomes for CAVHIS patients. The health inclusion HCSW will support the nursing and medical team to foster a culture of change towards health inequalities/injustices and advocate for accessible health care. The health inclusion HCSW will be dedicated, motivated, compassionate, adaptable and flexible and they will have the professional confidence to work in an unpredictable and fast paced environment.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
