GP with Special Interest in Diabetes, Community Diabetes Hub

Job summary

The community diabetes team in Cardiff and Vale UHB is expanding to develop a mutli-disciplinary Community Diabetes Hub. The team will consist of Community Diabetes Specialist Nurses, Dietitians, Podiatrists, a Community Diabetes consultant session and a GPwSI. The team will see patients referred from primary care, secondary care and other sources. There will be a strong focus on education and self-management, as well as providing an intermediate care service and keeping people cared for in the community wherever appropriate.

This role would allow development or expansion of an area of special interest in diabetes. As well as direct clinical contact, it will entail triaging of the referrals that come from primary care colleagues across CAV, allocating referrals to secondary care or the community hub as appropriate, and returning referrals with advice where suitable alongside signposting to Health Pathways. The clinical role will evolve alongside the Community Hub team, but will consist of regular MDT case discussions, joint clinics alongside MDT colleagues and face to face or telephone appointments. The role will include education and upskilling of community colleagues such as district nurses and primary care teams. The clinician will be responsible for their decisions about patient care but be working within a supportive clinical environment.

Main duties of the job

Main Duties and Responsibilities

Support the Community Diabetes Hub with:

  • Triage of referrals from primary and community care to diabetes services
  • Regular MDT cased based discussions of complex cases (may be virtually or face to face)
  • Face to Face / telephone patient facing diabetes clinics alongside MDT community diabetes team colleagues.
  • Supporting the diabetes community hub team in upskilling primary and community HCPs to deliver optimal diabetes care
  • Support the medical element of the diabetes remission programme
  • Provide support to the MDT, encouraging team working and good working relationships.
  • Work collaboratively with other colleagues across the UHB to ensure the delivery of research and development activity within the research governance framework and within the UHB's plan for research and development.

Performance Appraisal

Performance will be appraised and objectives agreed with the Diabetes Clinical Lead, and Supervising Consultant, in line with the UHB's Performance and Management Accountability Framework.
