Job summary
Based on its award-winning Rehabilitation Model, Cardiff and Vale UHB is developing a 'Live Well Service' to meet the needs of people with long term medical conditions. We are looking for a dynamic, innovative and experienced allied health professional to lead this multidisciplinary team and to shape and drive the development of this service taking our vision to its full potential. This is a highly collaborative post - working with our population to co-produce and deliver services; working with professional colleagues, managers and leaders within the UHB; and working to ensure we can demonstrate the impact of recovery focused approaches in the management of long-term medical conditions. This post is a unique opportunity to deliver a truly biopsychosocial approach, innovate across traditional boundaries and utilise community power.
Main duties of the job
The AHP Strategic Lead will be responsible for delivering our co-produced vision for the long-term conditions service - 'Live Well'. The clinical model is based on the Cardiff and Vale Rehabilitation Model and this role involves leadership of the service including service design, governance and developing clinical pathways. Innovation and co-production with partners, to enhance the offer is essential. Collaboration with peers across Wales is encouraged.
The post holder will line manage team members in collaboration with professional service leads supporting development within the service. Innovative workforce models to meet the needs of this population most effectively are encouraged. Co-production and partnership is essential through community presence to create and support new connections and wider engagement. There is clinical intervention for those with long term medical conditions including individual and group work, consultation to others, and supervision of colleagues.
The role involves education to support colleagues beyond the 'Live Well' team to develop their recovery focussed practice and embed a co-production ethos in our services.
There is a strong service improvement aspect to this role, demonstrating and promoting its value is essential. There are opportunities for research and audit.
