Job summary
St David's Hospital specialises in rehabilitation and complex care planning under the 'Discharge to Assess' model of care, which has been pioneering within NHS Wales and leading the way. You will be part of an efficient and effective Multi-disciplinary team, which includes volunteers and third sector, who are all striving to ensure that there is a person centered care plan and assessment to aid rehabilitate and support discharge home.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
This is an exciting opportunity to join a forward thinking team promoting 'homefirst' approach to delivering and meeting the needs of our patients.
Medicine Clinical board will be able to support your development, with dedicated Practice Development nurses, as well as a set program for all newly qualified staff.
