Job summary
An exciting opportunity has arisen for an experienced qualified nurse to join the Community Addictions Unit working within the Alcohol Team.
The post will encompass both the provision and application of Community Alcohol Detox procedures and key working responsibilities.
The suitable candidate would be required to, manage, monitor and support individuals through the process of medically assisted detox from alcohol within their own homes.
The role would also involve key working responsibilities for a small caseload of individuals with a diagnosis of alcohol dependency, This will involve assessment, care planning and treatment incorporating pharmaceutical and psychosocial intervention to support recovery.
Due to the community based nature of this position, a full driving licence and access to a vehicle is essential.
Main duties of the job
The main duties of the job will include: -
Taking responsibility for the application of medically assisted alcohol detox procedures within the clients home through health monitoring and medication administration and risk management.
Managinga small caseload of individuals with a diagnosis of alcohol dependency.
Conducting detailed biopsychosocial clinical assessments.
Incorporating patient centred care approaches to care planning and treatment pathways.
Apply brief intervention to support change.
Work from a context of harm reduction principles.
Liaison with a wide range of professionals to reduce risk and promote recovery.Providing supervision and support to junior staff and keyworker assistants.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
