Job summary

An exciting opportunity has arisen for a motivated and forward thinking mental health nurse with an interest in the mental health of older people to build upon existing foundations and further develop the Care Homes Liaison Service.

Care Home Liaison Teams' role is to promote mental health and wellbeing of residents in care homes through timely assessments and interventions that include diagnosis, psychological work, medication review, education and training. The ultimate aim is to maintain placements and prevent hospital admission.

Bridging the gap between primary care and secondary mental health services, the MHSOP Care Homes Liaison Team will enhance the integration between physical, mental health and social care in residential and nursing homes by working collaboratively with providers.

As leader of this MDT you will establish strong links with the residential and nursing homes within Cardiff and the Vale of Glamorgan in order to identify the demands upon the service and suggest how these can best be met keeping the older person at the centre of care provision and promoting person-centred and least restrictive care.

Key skills and knowledge; Clinical expertise in the mental health needs of older people, team leadership, training, service development.

Operational hours are 8am-4.30pm Monday to Friday.

Main duties of the job

The post holder will be responsible as a designated team lead for delivering evidence-based holistic care to patients within the context of a multi-disciplinary service.

The post holder will have clinical, procedural and budget holder responsibility for the service and will be responsible for scoping the service and developing care pathways in collaboration with the MDT and Directorate Management Team.

The post holder will supervise the day-to-day work of the clinical team, and provide expertise and guidance in complex and high-risk cases, in line with UHB policies and procedures.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
