Job summary

We are excited to recruit to a range of professionals to be part of a new multi-agency and multi-professional integrated urgent response service within Cardiff and Vale UHB.

Safe@home is a new multi-agency and multi-professional integrated urgent response service which addresses a current gap in our range of intermediate care services. Its aim is to provide an immediate and safe alternative to ambulance conveyance, EU attendance and admission to hospital when it is safe and appropriate to do so.

The new service is being developed for Cardiff and Vale drawing upon learning from established services across Wales and England. We aim to introduce and test the model as a new multi-agency, multi-professional approach to provide early senior clinical intervention and support in the community.

The Community nursing role will form part of the core response team, working with and alongside a range of clinicians in the community.

You will be required to travel throughout the region on a daily basis, using your clinical skills to undertake comprehensive assessments and provide acute clinical care and interventions to patients. In undertaking your role, you will be pivotal in ensuring that care plans support and enable patients to remain safe in their usual place of residence.

Main duties of the job

This is an exciting opportunity to join the newly created Safe@Home Service within Cardiff and Vale University Health Board. We are recruiting to Band 6 Community Nurse posts and offer this to experienced nurses with enhanced clinical skills transferrable to the community setting. Community/primary care experience is desirable but not essential.

The right candidate for this post will need to display excellent communication skills, problem solving and documentation with attention to detail and the ability to manage challenging and changing situations. They will be expected to work flexibly across Cardiff and the Vale of Glamorgan and will visit patients in their own homes utilising their clinical knowledge and nursing skills. They will also need to have a willingness to learn and be involved in service developments.

The post will involve working in partnership with the Community Resource Teams, District Nursing and other community services and collaboratively with related specialities across the UHB, and also Local Authority colleagues.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
