Smoking Cessation Specialist - Maternity

Job summary
This post holder is responsible for delivering a bespoke and flexible smoking cessation service to pregnant women within Cardiff and Vale UHB geographical area. As part of this service, the post holder will provide intensive behavioural support and pharmacological advice to smokers and others in the household who wish to stop smoking.
This post is a fixed term/secondment for 8 months to meet the needs of the service.

Main duties of the job
The post holder is responsible for working autonomously with patients to develop a treatment plan in order to deliver the support and help them stop smoking. This will involve working collaboratively with other clinical staff such as Midwives, Consultants and Health Visitors to identify patients and ensure that they are provided with the adequate pharmacotherapy.
The post holder is responsible for planning and organising their own workload effectively, this will include the management of a caseload. The post holder will be responsible for maintaining and inputting patient records into a digital database and ensuring that referrals are processed and patients are contacted within the adequate target time frame. The post holder will also work with clinical staff in the community and hospital to promote the service and support them to carry out brief intervention with patients. The post holder will work within standard operating procedures and use their own initiative when delivering information to clients.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
