Job summary

We have an exciting opportunity for a Band 6 Acquired Brain Injury Practitioner (Allied Health Professional or Nurse Practitioner) to work within the Specialist, Regional Community Brain Injury Team. We provide specialised neuro-rehabilitation to adults living in Cardiff & Vale and parts of Cwm Taf Morgannwg. The post-holder will play a key role in the co-ordination of rehabilitation, and will be involved in assessment and triage, intervention and liaison.

We manage a caseload of complex patients, provide education, support and clinical advice for patients, their families and the MDT to facilitate rehabilitation. We utilise advanced specialist skills and extensive knowledge base to undertake specialist assessments via a patient-centred approach.

Experience of working with patients with complex acquired brain injury would be beneficial.

We are seeking an enthusiastic and highly motivated, individual with excellent communication skills, effective teamworking and a passion for service development and improvement to join our team.

Main duties of the job

To devise, implement and follow up specialist treatments and rehabilitation across the spectrum for patients with Acquired Brain Injury. To be a skilled practitioner providing clinical care using an extensive theoretical knowledge base and practical experience to underpin the role, while acting as a role model for the delivery of care. To play a key role in the co-ordination of patients' neuro-rehabilitation and acting as a resource within the team and to broader services. To act as a resource for signposting and liaising with other services, ensuring holistic joined-up care.

To deliver specialist educational programmes to a range of healthcare professionals in relation to Acquired Brain Injury including medical, nursing and Allied Health Professional staff.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
