Job summary
Based on its award-winning Rehabilitation Model, Cardiff and Vale UHB is developing a 'Live Well Service' to meet the needs of people with long term medical conditions. This service will develop our long-Covid service widening access to people with other medical and long-term conditions with similar symptoms and needs including ME and CFS for example.
We are looking for a dynamic, innovative and experienced allied health professional to provide clinical leadership to a multidisciplinary team developing and delivering recovery focused interventions across the four tiers of the rehabilitation model.
This is a highly collaborative post - working with our population to co-produce and deliver services; working with professional colleagues within the UHB; and ensuring we demonstrate the impact of recovery focused approaches in the management of long-term medical conditions. This post is a unique opportunity to deliver a truly biopsychosocial approach and innovate in clinical practice.
Main duties of the job
The Clinical Lead will provide clinical leadership to the multidisciplinary team developing and delivering recovery focused interventions across the four tiers of the rehabilitation model to meet the needs of people with long term medical conditions including individual work, group work, consultation to others, and supervision of colleagues.
The post holder will foster recovery focussed practice utilising the full range of resources to promote personal wellbeing and self-management.
Partnership working is essential through co-production, fostering multidisciplinary and multi-agency relationships and drawing upon community assets.
The post holder will line manage and develop team members in close collaboration with professional service leads. The role involves training and education supporting colleagues beyond the 'Live Well' team in developing the way we deliver our services. The role will actively support practice-based education.
There is a significant service development aspect to this role with opportunities for research and audit. Demonstrating value and benefits realisation is crucial.
