Job summary
Based on its award-winning Rehabilitation Model, Cardiff and Vale UHB is developing a 'Live Well Service' to meet the needs of people with long term medical conditions. We are looking for a dynamic, innovative and experienced physiotherapist to join this inter-disciplinary team. This post is a unique opportunity to deliver a truly biopsychosocial approach, innovate across traditional boundaries and utilsing community power.

Main duties of the job
The role involves specific clinical input for those with long term medical conditions including individual work, group work, consultation, close collaboration and supervision of colleagues.
The post holder will have significant post registration experience, including working at a senior level in the the specialist area with clinical management experience.
The physiotherapist will work flexibly, autonomously and collaboratively with fellow AHP team members to provide holistic and transdiagnostic interventions supporting self care management and behavioural change.
The post holder will carry a clinical caseload and act as a resource providing advice and support to other physiotherapists and AHPs within collaborating services across the UHB. They will lead professionally on physiotherapy related issues, within the Long Term Condition Management clinical area or pathway. This will include patients with multi-pathology and complex needs.
They may be required to work their normal contracted hours flexibly in a variety of different working patterns in response to the needs of the service. For example, this could include working their contracted hours as part of a 7-day job plan or working their hours over a pattern that includes extended days and unsocial hours.
There is a strong service development and improvement aspect to this role with opportunities for research and audit.
