Job summary

The post holder will be working within a team providing therapeutic care within a holistic philosophy for individuals experiencing mental health crisis.

The function of the Crisis Recovery Unit is to prevent hospital admission, to facilitate early discharge from the acute hospital setting and to support family and carers. This is achieved by working in collaboration with the Crisis Team, hospital and community based multi agency resources.

Main duties of the job

To provide therapeutic care delivery to clients who are in mental health crisis, to deliver evidenced based mental health support in a compassionate and person-centred way, that will enable the client to move towards their own level of recovery.

To work within a multi-disciplinary team that is able to support clients in a psycho-social and medical framework.

The ability to speak Welsh is desirable for this role but not essential and all candidates would be treated equally and are welcome to apply.

Development of Person Centred Nursing Intervention Plans for patients who meet the criteria for the Crisis Resolution and Home Treatment Team and be responsible for assessing individual needs, planning care, implementing care consistently, and evaluating the outcomes of care under the supervision of the Team Leader.

  • Supporting/Mentoring Nursing Students.

Supporting, signposting clients and carers needs for appropriate community based care and help, to maintain patient dignity and self-esteem. Promote maximum independence through preventing social isolation.

Liaising with Social Services, developing relationships with local primary care providers, health agencies, voluntary agencies to promote partnerships in primary care.

To support clients and carers by providing Evidenced based Psychological Therapeutic Interventions.

Assist the client and carer by supporting and educating, to enable carers to provide appropriate care. Monitor prescribed treatments at home or in the team base, including medication and psycho-social interventions, promoting health education at every opportunity.

Participate in attendance at multi-disciplinary reviews and meetings in relation to admission and discharge planning, and provide or receive feedback on all relevant information.

To administer prescribed medications including Clozapine Initiation and Depot Injections.

Keep accurate records in keeping with local and statutory bodies and maintain strict confidentiality at all times. Also keep an effective recording system for visits, travelling etc. for audit, statistical and research purposes.

Teach and supervise learners in all aspects of the Community Mental Health Nurses role and maintain close liaison with local education providers.

Participate in personal development opportunities, maintain and develop knowledge and skills through the appraisal process, and keep professionally updated at all times, undertaking duties in accordance with best practice and PREP requirements.

Actively participate in management supervision from a senior band 6 nurse or the Team Manager.

Actively participate in clinical supervision.
