The IRIS are a nurse led Community Rehabilitation Team working in Cardiff and the Vale of Glamorgan. The IRIS team work in partnership with tenants, CMHT's, GP Practices, housing associations, and private landlords. The service operates 7 days a week with the aim being to promote rehabilitation and recovery.

The role encompasses involvement in intensive assessment process, individualized rehabilitation programmes and goal setting towards obtaining optimum levels of independence for clients. Promoting self development and skills learning through vocational, recreational and employment strategies.

The post holder will work as part of a team to deliver a high standard of care which is least restrictive and promoting recovery. They will carry out direct patient care activities under the direction of qualified member of the IRIS and be expected to work autonomously and mostly without direct supervision from qualified staff.

Main duties of the job

An opportunity has arisen for a suitably experienced individual who is highly motivated, innovative, enthusiastic and passionate about service user centred care to work in the IRIS Team based at Riverside H/C Cardiff.

The post-holder will assist in the provision of mental health services for Rehabilitation and Rehab services and clinically related clerical duties as appropriate. The support worker will carry out direct patient care activities under the direction of qualified member of the IRIS and be expected to work autonomously and mostly without direct supervision from qualified staff.

The post holder will be spending a large amount of time working autonomously which could involve supporting the client to attend OPA or attend to their ADLs by escorting into their local areas to support day to day tasks like shopping.

You will have the flexibility to meet the needs of the service to work shifts (no night shifts) and the service operates 7 days a week, 365 days per year.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
