Job summary
We are looking to recruit a new GP Community Director (Cluster Lead) to join the Vale Locality Management Team and take lead of the Eastern Vale Cluster.
Working within the Primary, Community and Intermediate Care Clinical Board, the Cluster Community Director will undertake a pivotal role in developing the Primary Care Cluster in line with the transformation agenda set by Welsh Government. Working closely with local primary care providers, social care services and hospital and community based services, the CD will be influential in the development of the accelerated cluster in order to meet the new models of care and development of the Cluster.
As part of the role you will be expected to provide clinical advice/leadership on cluster issues. You will need to maximise and maintain the communication network within your cluster practices including assessing and addressing local CPD issues and facilitating the evolution of Cluster Network Development meetings to become mutual business clinical/learning forums. You will also co-ordinate and chair regular cluster CDS sessions as well as lead on the provision of primary care clinical pathway development. You will take a lead role in developing the maturity of the cluster, with specialist support being provided from within the PCIC Clinical Board.
You will be supported in your role by the Locality Manager to whom you will be operationally responsible, with additional support being provided via a Cluster Development Manager.
Main duties of the job
Following the launch of the Welsh Government Accelerated Cluster Development Programme, Clusters in Wales are now required to work in a far more integrated way- ie working with all independent contractors, councils and the third sector to review population need assessments and identify opportunities to address service gaps.
The Cluster Community Director has a pivotal role in harnessing the ideas and views of these wider stakeholders and driving forward with the aspirations and plans for the Cluster. They will lead local Cluster meetings and also engage in strategic discussions at the Pan Cluster Planning Group level, where there will be opportunities to influence service planning and investment at an organisational level. The Community Director will also lead on developing the autonomy of the Cluster.
The Community Director will be supported by the Locality Manager and a Cluster Development Manager. There will be an opportunity for peer support and funded training and education programmes to support induction into the role.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
