Job summary
To provide clinical leadership in the development of multi-disciplinary working at cluster level.
To autonomously manage a caseload of clients with complex needs, using evidence based / client centered principles to assess, plan, implement and evaluate interventions in primary and secondary care settings to include home visits and community facilities
To provide leadership for a clinical and administration team to include responsibility for appraisal, supervision, induction, training and education
To participate in the planning, development and evaluation of Occupational Therapy services within a designated area/team, holding responsibility for defined projects.
To contribute to the maintenance and development of the UHB Occupational Therapy Service.
To demonstrate a commitment to the education of Occupational Therapy students ensuring the maintenance of clinical standards through regular supervision of students on practice placement.
To work in collaboration with the multi-disciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface
To maintain clinical records in accordance with legal requirements and professional standards.
Main duties of the job
In terms of specific details of the role, the postholder will be required to:-
- chair the MDT meetings and provide professional leadership/guidance where required
-act as the clinical conduit for coordination of required actions which fall out of the MDT- which may include patient facing assessments/home visits
- act as clinical link with other relevant health services eg the community resource team/specialist services - with a focus on supporting hospital discharge, avoiding admission
- assist with the set- up of the coordination hub and provide day to day operational oversight on behalf of the Locality Manager, including line management of junior admin staff
- work with other professionals allocated to the coordination hub to ensure a timely and prudent response to enquiries received from practices/patients
-support with performance management
