Job summary

An exciting opportunity has arisen for a dynamic and innovative Occupational Therapist to independently plan, organise, lead and evaluate the occupational therapy service within the Continuing Healthcare (CHC) service in the Vale of Glamorgan; working with adults with complex needs who require assessment and provision within the NHS CHC criteria.

This post will be based within Vale Community Resource Service in Barry but will also work closely with local authority to provide seamless support to those transferring from social funded support to meeting CHC criteria.

The post holder will assess, plan, implement and evaluate interventions for adults who live in residential nursing homes, private nursing homes, own homes and supported living accommodation

The post holder will contribute to the planning, development and evaluation of Continuing Healthcare services across the Vale of Glamorgan.

Main duties of the job

The main duties of the role include . . .

  • Autonomously managing a caseload of clients with complex needs, using evidence based / client centered principles to assess, plan, implement and evaluate interventions in primary care settings to include home visits and community facilities
  • Providing leadership for a clinical team to include responsibility for appraisal, supervision, induction, training and education
  • Participating in the planning, development and evaluation of Occupational Therapy services within Vale of Glamorgan, holding responsibility for defined projects.
  • Contributing to the maintenance and development of the Occupational Therapy services within Vale of Glamorgan.
  • Demonstrating a commitment to the education of Occupational Therapy students ensuring the maintenance of clinical standards through regular supervision of students on practice placement.
  • Working in collaboration with the multi-disciplinary team and with primary care, voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface
  • Maintaining clinical records in accordance with legal requirements and professional standard
