Job summary
A unique and exciting opportunity is available for a patient focused, motivated, enthusiastic and dynamic individual to join the Patient at Risk Team working across Cardiff and Vale UHB.
Do you aspire to make a real positive contribution to patient safety and experience? Are you a registered nurse, with experience in the assessment, planning and management of the deteriorating patient with excellent communication, teaching and clinical skills?
Main duties of the job
The Patient at risk team practitioner is required to work alongside the Multi-disciplinary team, working closely with nursing staff, primary teams and the P@RT medical team to provide comprehensive care to all of the needs of a deteriorating ward patient including diagnostic review, timely escalation to higher levels of care, instigation of treatment limitations or palliation.
Do you enjoy and have experience in teaching? The P@RT practitioner will be instrumental in ensuring the delivery of high quality training to all ward staff, MDT staff, undergraduates and postgraduates around the recognition and management of acutely ill ward patients.
We are looking for innovative, forward thinking nurses who will be proactive in conjunction with other P@RT practitioners developing this role further in response to challenging healthcare needs and advances in practice.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
