Gibraltar is a British Overseas Territory at the southern tip of the Iberian Peninsula, easily reached in just over 2 hours (flying time) from London, Bristol or Manchester airports. The post is based at the St. Bernards Hospital located by the waterfront, overlooking the straits of Gibraltar & Northern Africa.The area is perfect for those wishing to explore Southern Spain, Portugal and North Africa. English is the official main language with Spanish widely spoken.
Applications are invited from experienced Junior Doctors in Adult General Internal Medicine (GIM) for appointment to the above vacancy within the Gibraltar Health Authority (GHA). The GHA is organised and modelled on the UK NHS and follows traditions of British medical practice.
Given the small nature of the teams and the unselected nature of the GIM take, this post provides an excellent opportunity for doctors to be exposed to the full panoply of presentations, gain experience and confidence in their management and to learn and up-skill in procedures (e.g. chest drain insertion, lumber puncture, paracentesis) whilst benefitting from close supervision by the consultant body.
Main duties of the job
The Medical Team comprises of 10, largely single handed, consultant specialists, 6 of whom currently contribute to a 1 in 6 General Internal Medicine (GIM) Consultant on call rota.
These specialties comprises of the following fields:
- Cardiology (2 Consultants and an Associate Specialist)
- Diabetes & Endocrinology & GIM
- Gastroenterology & GIM
- Haematology
- Nephrology & GIM
- Older Peoples Medicine (OPM) & GIM (2 Consultants)
- Respiratory & GIM
- Rheumatology
- Neurology currently supported by a visiting service (remote and face to face) supported by an in-house specialty doctor and one of the OPM consultants with an interest in neurology
The 10 consultants are supported by up to 10 Clinical Fellows (CFs, formally known as Non-Consultant Hospital Doctors/NCHDs) including the successful appointee. In the current configuration, 3 of the more junior CFs provide cover for the acute medical ward 9-5 Mon-Fri and 9-1 at weekends on rotation. The remaining 7 participate in a EWTD-compliant full-shift rota covering admissions, medical patients on CCU and other areas of responsibility within the medical directorate.
The consultants provide 24/7 non-resident on call and can be readily contacted for remote or in-person advice as needed. Post-take ward rounds are undertaken twice daily 7 days a week with the consultant available at any time for advice and/or bedside review.

The Consultants work primarily from a mixed Medical Ward and a Critical Care Unit which serves as an Intensive Care, Coronary Care and a Medical High Dependency Unit. The medical unit runs in an orthodox manner modelled on the NHS with daily ward rounds, outpatient clinics, and ward work. There are regular internal CPD and Morbidity and Mortality sessions and wider organizational-level CPD sessions which the successful applicant will be encouraged to actively participate in.
The Outpatient facilities and the Consultant offices are located in St Bernards Hospital on the Fifth Floor in the Medical Investigation Unit. This unit also houses the Endoscopy unit, bronchoscopy, as well as cardiology and respiratory investigation units and the Haemodialysis Unit.
The successful candidate will play an important role in the day to day work associated with the care of medical inpatients and the running of the department closely supervised by the Consultants. These roles include:
- In patient management, including daily ward round and ongoing patient care for patients under the care of the medical team
- Emergency on-call covering acute admissions via A&E and GP referrals
- Seeing and reviewing referrals on the surgical ward
- Undertaking ward based procedures (subject to competence) e.g. lumbar puncture, abdominocentesis, thoracocentesis, chest/abdominal drain insertion
We are a small team and pride ourselves on Team Work and flexibility. A willingness to cross cover and assist colleagues depending on the varying clinical demands on a given day is a key requirement of this role.
In patient activity will include:
- Responsibility for the daily management of patients under the care of the medical team
- Assessing medical patients referred from A&E and primary care and clerking urgent admissions from clinic. Such assessment will usually include carrying out all functions related to acute admissions including clerking, initiation of treatment, requesting and reviewing investigations and ongoing care. For those not being admitted, preparing relevant documentation to record and communicate the encounter e.g. discharge letter, entry into the electronic GP records, completion of the A&E electronic notes system
- Attends thrice weekly consultant ward rounds and carrying out and documenting any other ward rounds as requested by the consultant
- Post take ward rounds to review acute admissions when on call and arranging all necessary investigations and treatment
- Ensuring appropriate and prompt patient management of patients under the care of the Medical Team
- Documentation of all ward rounds and clinical decisions in the clinical notes
- Initiation of appropriate investigations and treatments as per the unit protocols and standard clinical practice
- All clinical investigations should be reviewed, checked and recorded in the clinical notes. Abnormal results should be highlighted and acted upon. Where necessary, such results should be brought to the attention of the appropriate consultant
- Out of hours, reviewing unwell patients with acute medical issues at the direction of the nursing staff and other clinical teams as appropriate
- Clinical Fellows are expected to work in CCU to manage medical patients under the supervision of their medical consultant with assistance being provided by the resident Intensivist if required
- Patient discharges should be coordinated by the CFs and the appropriate documentation completed; Clinical Fellow General Medicine May 2024 Each patient should have a typed written discharge summary produced and sent to the GP with a copy placed in the clinical notes prior to discharge
- Liaise with Bed Manager on the safe transfer and discharge of patients
- To undertake such duties as may be required from time to time that are consistent with the responsibilities of the grade and the needs of the service
