This is a Clinical Fellow (NCHD) for a General Surgery role. This post will provide you with the opportunity to gain clinical experience in Surgery at the level of CT2 or ST3 (Junior Level). This is not a recognized training post but provides excellent opportunity for research and training. Our Consultants are registered on ISCP and they are used to supervise trainees and help them to get a training post. Most of the recent fellows have obtained training post after their placement in Gibraltar. This post will be a GHA appointment for 12 months with a focus on Surgery during normal working hours at St Bernards Hospital.
Main duties of the job
- The Clinical Fellow will be expected to provide resident cover primarily to the Surgical Team during working hours.
- You will be accountable to the Surgical Consultants and their patients. After hours and when on call, you will be expected to deal with all surgical inpatients and surgical admissions.
- You will also be required to cover all other hospital inpatients and will be the first port of call for routine enquiries.
- You will be expected to deal with issues within your clinical competence and escalate to senior staff if you are unsure (including Obstetrics & Gynaecology, Maxfax, ENT and Urology).
- To undertake such duties as may be required from time to time that are consistent with the responsibilities of the grade and the needs of the service.

Services are mainly provided from St. Bernards Hospital. The hospital also collaborates with international medical institutions for specialised treatment and support.

- The successful candidate will pay an important role in the day to day work associated with the care of St Bernards inpatients.
- Admitting acute surgical patients from A&E and Urgent /Fast Access clinics.
- Admitting of elective surgical patients.
- There are daily ward rounds starting at 08:30. These are led by the surgical consultants and attended by the surgical juniors (as directed by the weekly rota). From time to time the CF is required to perform the ward rounds themselves and refer to the consultants for advice and direction.
- The CF is required to perform an evening ward round in order to collate results and ensure that management changes are not required.
- Ward rounds should include all outlying patients and may include appropriate transfer of these patients to the surgical ward if necessary.
- Ensures appropriate and prompt patient management of surgical inpatients both in the Surgery ward and other wards.
- Ensures referral to the medical doctors for the management of surgical patients with complex medical histories.
- Documentation of all ward rounds and clinical decisions must be made in the clinical notes.
- Appropriate investigation and treatment should be initiated as per the unit protocols and standard clinical practice.
- All clinical investigative results should be reviewed, checked and recorded in the clinical notes. Abnormal results should be highlighted and acted upon. Such results should be brought to the attention of the appropriate consultant.
- Patient discharges should be coordinated by the CF and the appropriate documentation completed.
- Each patient should have a dictated, type written discharge summary produced and sent to the GP with a copy placed in the clinical notes. Discharge summaries will be dictated within a week of the patient leaving the ward.
- Liaise with Bed manager on the transfer and discharge of general surgery patients.
- Attend theatre sessions as per the weekly rota.
- To have responsibility that all patients are adequately prepared for the operating room and that the relevant radiographic investigations are available.
- To mark patients as per standard pre-operative marking protocols.
- To assist at and perform operations as directed by the consultant in charge of the operating list.
- To attend post-operative ward rounds with the consultant in charge and will liaise with the on-call CF regarding newly operated patients.
