The Optalis Short-Term Support and Rehabilitation Team (STS&R) provides reablement care and support to people across the Royal Borough of Maidenhead and Windsor.
Reablement is a short-term, home-based support service that helps individuals regain skills, abilities, and confidence to manage daily tasks and live as independently as possible. It's a service we provide to people after an illness, injury, or hospital stay and is different from traditional care because instead of doing things for someone, reablement focuses on supporting people to do things for themselves again. With personalised recovery goals and plans agreed with our team of Occupational and Physio Therapists, reablement aims to improve independence by focusing on practical skills and activities.
Simply put, reablement is about helping someone get back to their usual routines - like washing, dressing, cooking, or moving around - so they can live as independently as possible.
Main duties of the job
We are recruiting for Rehabilitation Care Assistants to expand our current team and increase capacity for supporting customers back to their own homes.
As a Rehabilitation Care Assistant, you will support customers in their own homes with rebuilding their confidence and regaining their independence. Following a person-centred care plan created by our team of Occupational Therapists and Physiotherapists, you will engage and motivate customers to work toward their recovery goals. There will also be a need to support with care needs, such as personal care, moving and positioning, administering medication and some domestic duties.
Whilst the service is based out of Maidenhead Town Hall, this role is primarily visiting customers at various locations across the borough. It is essential that you are able to drive and have your own vehicle.
Working full-time will generally involve 4-5 shifts a week, covering a mix of AM (0700-1500) and PM (1400-2200) shifts, as well as working alternate weekends.
As our customers receive a maximum of six weeks support, your caseload will be varied and will change regularly, truly making no two weeks the same.

The role of the rehabilitation assistant is integral to the service and vital in enabling the service to support those that wish to remain at home for end of life care, prevent hospital admission and enable residents to leave hospital earlier to continue rehabilitation and recovery at home. The role is supported by a team of therapists guiding and educating in the delivery of physiotherapy and occupation therapy programmes.
Responsibilities include;
- To deliver safe and skilled person centred support to residents of RBWM in their own homes. This can include personal care, meal preparation, medications and rehabilitation programmes.
- To provide detailed feedback to the therapy team/service coordinators and duty officers to enable the rehabilitation plans to be reviewed and updated as the service user progresses back to independence, the service provides equipment to allow the team to manage this.
- To report any deterioration and or changes in condition to enable appropriate medical interventions to take place, this may include liaising with district nurses, GPs, social workers and other allied health professionals.
- To confidently use initiative and decision-making skills in order to keep the service users safe and free from harm in the community.
- To participate fully in weekly and monthly team meetings, supervision and appraisal process.
- To work with families and carers to explain, and support them to understand, the rehabilitation process.
