Job summary

Under the leadership and management of the Occupational Therapist, the Reablement Referral Coordinators, will support the implementation of a new model of service delivery for Reablement, covering Cornwall County.

Comprising of an initial up to 3 day assessment to assess for Reablement potential and a further continuation of up to 21 days to reach identified Reablement goals.The Intake model for community services will receive all referrals initially into the central hub, whereby all individuals for whom care and support at home is contemplated will be assessed by the STEPS Reablement referral coordinator for their Reablement potential.

Adopting a risk-based approach, an initial screening and assessment will be offered to new customers, to include that of TEC enabled care assessment within the first three days of receipt of referral, to identify needs and risk and include the production on either an initial care and support plan or signposting to other community or voluntary based services.

The model operates a deselect option in relation to the provision; of End of Life and Advanced dementia. Following the initial three days assessment, whereby Reablement potential has been identified, the continuation of assessment and a detailed Reablement plan, including risks will be undertaken by Community Care Supervisors within the wider STEPS teams localities, ensuring the service is target driven along with efficient use of capacity.

Receive and investigate complaints, accidents and incidents according to procedure and ensure that any safeguarding issues are dealt with expeditiously. Close liaison with the Corserv Cares, Hospital Discharge Co-ordinator, based at RCHT to ensure capacity is prioritised for medically fit patients requiring discharge to Corserv Care, through either the STEPS or Care & Support Teams to provide packages of care.

Assess the potential to improve outcomes/lives gathering information from a range of sources, including adults, carers; and relevant others; other agencies, and other people within the person's wider support network. Promote independence by offering information and advice, signposting to other agencies as appropriate, and prompting contact with social and community resources. Accurately record assessments, reviews and support plans, ensuring a copy is provided to the person concerned.

Maintain care records, keeping them up to date so that they can provide concise and accurate information about circumstances and plans within the limits of data protection. Promote and facilitate effective communication with families, carers and individuals and other agencies to ensure the views and wishes of individuals and carers are given appropriate consideration and are central to all decision making. Seek advice from colleagues, other professionals, and line manager where this supports safe and effective decision-making.

Maintain awareness of policy and procedures to safeguard adults at risk of abuse or neglect, alerting others to risks as appropriate. Attend training courses as required and maintain own professional development and competency including having responsibility for own self development on a continual basis. Operate with due regard to Corservs confidentiality policies, the Data Protection Act and best practice and work at all times within the requirements of Health and Safety legislation.

To have an awareness of, and be able to contribute to, mental capacity assessments in line with the Mental Capacity Act. Promote assistive technologies, minor adaptations and equipment required to maintain/enhance/develop independence.
