Job summary

Working within Middlesbrough Council, our team helps those experiencing problems with substance use, domestic abuse, and homelessness.

We are looking for a dedicated and compassionate Nurse Associate to join our team, helping those who experience problems with substance use.

As a Nurse Associate, you will play a vital role in delivering high-quality care and support to clients that access our service.

Under the guidance of your line manager and working alongside the wider teams you will:

Provide care and support to clients in line with their recovery care plan.

Undertake clinical tasks including (but not limited to):

  • ECGS
  • Health assessments
  • Alcohol assessments
  • Titrations
  • Phlebotomy
  • Buvidal administration
  • Wound care

Contribute to the assessment of clients needs, care plans, and processes.

Update and maintain client records.

Promote health and wellbeing at all times, supporting clients to address their health needs.

Recognise and raise issues relating to safeguarding.

Have a strong commitment to continuous professional development.

Participate in multi-disciplinary team meetings, case conferences, and clinical supervision sessions to review service user progress and adjust care plans as necessary.

To be eligible you will:

Have a Nurse Associated Foundation degree or equivalent qualification recognised by the NMC.

Be a registered Nurse Associate on the NMC register.

2. To collaborate with the wider team and other services to assess client needs and develop individualised care plans.

3. To undertake a range of clinical tasks, ensuring adherence to service protocols and professional standards and regulations.

4. To monitor clients progress, evaluate responses to clinical interventions and adjust care plans as necessary.

5. To inform clients of treatment options and provide them with harm reduction strategies.

6. To identify and assess any risk/safeguarding concerns and raise with the appropriate channels.

7. To coordinate referrals to other healthcare providers and community resources as necessary.

8. To commit to continuous professional development.

9. To partake in ongoing quality improvement initiatives to enhance client care and outcomes.

10. To maintain accurate and timely documentation of all client interactions.

11. To understand and act in line with NMC professional standards for practice contained within The Code.

12. Understand requirements for NMC professional Revalidation.

We sit within the ACT model (Accessing Change Together) which also encompasses Homelessness, Housing services, Domestic Abuse and Welfare Rights. This new model supports an integrated approach to care for those needing to access multiple services. This new model supports an integrated approach to care for those needing to access multiple services. The Nurse Associate post aims to provide exceptional nursing care and support to the clients who access our service many of whom are vulnerable and/or have complex needs. You will work collaboratively with the wider ACT service (and/or any other relevant services) to assess patient needs, develop individualized care plans and implement clinical interventions.

You will deliver compassionate, trauma-informed care undertaking a range of clinical tasks whilst also monitoring patients progress, making referrals to other healthcare providers where appropriate, updating client records and identifying and raising any safeguarding concerns. Additionally, you will play a key role in coordinating clients care with other healthcare providers/community resources and participating in ongoing quality improvement initiatives as well as continuing with your own continuous professional development.

  • Experience of providing nursing care as part of a multi-disciplinary team.
  • Proficiency in ICT and experience of inputting and maintaining client records on a caseload management system.
  • Ability to communicate effectively (written, verbal, and non-verbal communication) with clients, other professionals, and all members of the multi-disciplinary team.
  • Effective time management and the ability to organise and prioritise own delegated workload.

Desirable
