Hartlepool Borough Council are seeking to recruit an enthusiastic and motivated registered nurse to work within our 0-19 service, predominantly with our Health Visitors delivering the Health Child Programme and Public Health agenda.

Main duties of the job

You will work in the community supporting families in their own homes, Family Hubs, community and Education settings. Your role will include delivering health promotion, universal and targeted interventions to individuals and groups as part of the 0-19 Early Help Team.

You will be supported to fulfil your role through induction, training, shadowing and a tailored preceptorship package.

As part of the role there is the options to train as Specialist Community Public Health Nurse in the future. Purpose of Post

To work as part of an integrated locality early help team providing an effective high quality service to the local population ages 0-11 and their families.

To be a member of a Health Visiting Team and undertake duties as delegated by the Specialist Community Public health nurse

To provide a service which includes health surveillance, health promotion, support and advice, primarily to families with children under 6 years of age as part of the delivery of the Healthy Child Programme.

To work as part of the wider community nursing and primary healthcare team 0-19 team, liaising and working with other statutory and voluntary agencies as appropriate.

This post will prepare the staff nurse to undertake training in the future if they wish, to qualify as a Specialist Community Public Health Nurse.

Main Duties and Responsibilities

Work with parents and children to promote the physical,emotional and social development of children, particularly those who are disadvantaged.

Deliver coordinated interventions within the framework of The Healthy Child Programme as delegated.

To revew the health needs of the child and community involving the parent/carer and /or child in the process of meeting those health needs.

To monitor the health of the child and to support the parent/carer in promoting optimum health and development of the child.

To be aware of the deviations from the norm in health and behaviour and to intervene to protect vulnerable children by prompt action, by referral and working with other colleagues and agencies as part of a specialist team.

  • Deliver health promotion and improvement interventions to groups, children and families including, but not limited to, healthy weight, the prevention and reduction in the number of child accidents, Sudden Infant Death Syndrome, smoking, domestic abuse, post-natal depression, infections, heart disease and cancers across all ages groups.

To assist with Child Health Clinics to monitor the growth of babies and children, advising parents/carers on aspects of parenting and refer to the Health Visitor with any identified issues.

Undertake developmental assessments of children under the age of 6 years as directed by the Health Visitor.

To support families with children with SEND or complex medical needs.

Follow up attendance at A & E Departments or Minor Injuries Units as delegated by the Health Visitor

To visit and assess families with children under the age of 6 years who move into the area.

To be able to assess childrens health needs and signpost to local support service.

To support transition and school readiness.

To perform tasks which is commensurate to your role

  • To ensure that the highest standard of care is achieved at all times in the most cost effective manner.

  • Demonstrate up-to-date research-based, clinically effective practice, ensuring awareness of current legislation, education and audit.

  • Ensure accurate records are maintained in the Early Help Locality Teams, in accordance with local and NMC standards.

To assist in maintaining a healthy, safe and secure environment and to act in accordance with the NMC and the Councils policies and procedures.

To participate in risk assessment of clinical and non-clinical areas, and report accidents or near misses promptly and when required using the appropriate incident reporting tool, and cooperate with any investigation undertaken

Have the autonomy to make direct referrals to all agencies/disciplines/voluntary groups.

Contribute to a monthly Audits including records to assist in maintaining standards for records and record keeping in response to local and national standards and NMC guidelines.

To actively participate in agreed management processes e.g. supervision and appraisal

Present complex and sensitive information and contribute to decision making within a multi agency forum that includes parents and carers and produce timely and informative reports and summaries for e.g Social Services, Care Co-Ordination, GPs, School Nurses etc.

Mentor Children and Adult nurse students in order to provide a learning experience to gain a holistic understanding of the health and development of children within the family context and how this affects their individual needs and behaviour.

Support the development of the wider Multidisciplinary team and the Community Nursery Nurses, whilst also delegating identified tasks and pieces of work appropriately.

To maintain own level of clinical competence by reading relevant literature and taking opportunities for personal and professional development and undertake mandatory training

To participate in the identification of local health needs and the development of strategies to meet these

Communication and Relationship Building

To ensure good communication channels are maintained within the Health Visiting Team.

To work collaboratively with other statutory, voluntary and community organisations to facilitate service provision and access for families and individuals.

Populate own SystmOne ledger at least two weeks in advance for efficient allocation of work.

Effort, physical skills and working conditions:

Physical skills Carry materials and equipment, e.g. weighing scales, Leicester height measure, boxes of leaflets, etc in accordance with relevant health and safety policies.

Drive and hold a current driving licence and have access to a vehicle for work and to be able to travel efficiently around the local geographical area.

Provide evidence that the vehicle is insured for business use.

Move around effectively to undertake the roles and responsibilities of the post and have the accuracy and dexterity to participate in public health sessions such as immunisations, growth monitoring and screening.

There may be periods of time when sitting in one position will be required and on occasions it may be necessary to bend or kneel to effectively communicate with a child.

Plan, implement, deliver and evaluate episodes of care to children and young people adopting a flexible approach to meet individual needs.

The expectation is that the post holder is familiar with the relevant procedures and guidelines, all of which can be found on the Local Authority intranet.
