We are seeking a highly motivated Leader to Coordinate an extension to our Supported Living service within the community. This is a fantastic opportunity to make a real difference in the lives of the people we support, while driving quality and innovation across our services.

Main duties of the job

About the role

Coordinate support for two young adults living in their own home, ensuring the staff rota is in line with their needs.

Keep up to date and review relevant support plans, risk assessments and tools to support the staff team to deliver high quality support.

Liaise with families, professionals and key people with regards to day-to-day queries and updates.

Undertake regular quality assurance and gain feedback from staff, families and people we support.

To work with the people we support as and when required in order to build positive relationships, guide the team and be a present leader.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.

Interviews will be held as and when successful applicants are shortlisted.
