Job summary

We are seeking a newly qualified dedicated Occupational Therapist to join our dynamic and supportive Therapy Reablement Service. If you are looking for a role that enables you to make a real difference to peoples lives, please apply.

Main duties of the job

To provide a range of Occupational Therapy interventions to adults referred to the service. Focusing on enhancing individuals ability to undertake daily activities to maximise their independence and quality of life.

Job description

Job responsibilities

Main Responsibilities:

Hospital discharge and community support:

Work with individuals on a Discharge to Assess pathway to support a smooth transition from hospital to home supporting recovery and independence.

Support individuals living in the community who require therapy reablement to prevent hospital admission.

Strength-based assessment:

Undertake holistic and strength-based assessments, working with individuals to develop personalized goals and reablement plans.

Assess and arrange for provision of equipment and adaptions to maximise their independence and safety, including major adaptations where necessary.

Undertake risk assessments of complex moving and handling situations and develop management plans

Ensure the active involvement of service users and carers in assessments, the development of support plans and accessing Direct Payments

Identify solutions including assistive technologies, providing demonstrations to ensure these are used safely

Accurate and timely recording and reviews:

Maintain accurate and timely assessment and case records for all service users seen. Monitor and review support plans to ensure ongoing effectiveness and take action as appropriate.

Ensure systems are updated to support service delivery and the routine collection of statistical data.

Professional Support and guidance:

Supervise Occupational Therapy Assistants, providing support and guidance and professional specific guidance to Social Workers

Integrated working:

Work in an integrated way with colleagues and other health and social care professionals to endure the best and most seamless transition for service users. Fully participate and engage with developments in Adult Social Care, and your own development.

Experience

Essential

  • Experience in an adult social work/adult health environment
  • Experience of safeguarding policy and procedures
  • Understanding of the Care Act 2014 and the national agenda for health and social care

Desirable

  • Experience working within a Reablement team

Qualifications

Essential

  • Degree/Diploma in Occupational Therapy
  • HCPC registration

Desirable

  • Moving and Handling trusted assessor training

Skills

Essential

  • Able to make sound professional judgements in planning and
  • evaluating care
  • Able to devise reablement goals and plans, and implement these
