About the job.

Working within the Community Support Services you will have the opportunity to work in both supported living and day opportunities. Some evening/weekend and bank holiday work is required to meet the needs of the Community Support Service and its service users, however we are open to negotiation and will consider flexible working patterns to suit your life style including nights, days, evenings, weekends and will consider term time only options for the right applicants.

Due to the nature of the role, some elements of physical work such as moving and handling and delivering personal care will be required.

Basic pay of £10.79 - £10.98 per hour, Saturdays at £16.19 - £16.47 per hour, Sundays and bank holidays at £21.58 - £21.96 per hour. All hours worked above 37 hours per week paid at £21.96 per hour.

Main duties of the job

Previous experience in a similar role would be advantageous, however, we can offer you a comprehensive induction and a tailored training plan, with support to undertake the QCF Diploma and Level 2 Health and Social Care qualifications fully funded by us!

Community Support Worker Day Opportunities

Purpose of the Job:

To support adults with learning disabilities to participate in a range of activities using both service based and community resources as appropriate. To use person centred approaches to enable people to make their own choices and have control over the support they receive to access community, education, and employment opportunities. To promote social inclusion and contribute towards service development to meet individual and service outcomes.

Key responsibilities:

1. To support the development of person centred plans, ensuring people have meaningful and achievable outcomes that meet aspirations as well as needs

2. To promote independence by supporting people to make their own choices and decisions.

3. To support individuals with their personal care ensuring all tasks are carried out with respect for privacy and dignity and in line with needs, wishes and preferences as defined in the support plan.

4. To safeguard and promote the welfare of people in Rutland through the embedding of safeguarding processes and practice

5. To promote healthy life styles for individuals, liaising with other professionals and agencies as appropriate.

6. To undertake specific health related tasks and implement plans of support developed by members of the multi-disciplinary team following training.

Community Support Worker Supported Living

Purpose of the Job:

To support adults with learning disabilities to undertake tasks with/for the people using the service in line with their individual needs. To use person centred approaches to enable people to make their own choices and have control over the support they receive To promote social inclusion and contribute towards service development to meet individual and service outcomes Key responsibilities:

1. Assist individuals to adhere to their tenancy agreement and liaise with representatives from the Housing Association. Encourage individuals involvement in the day to day running of their home, and to increase independence skills

2. Support the development of person centred planning, ensuring each individuals plans has meaningful and achievable goals with positive outcomes that meet aspirations as well as needs

3. Actively support individuals in housing related tasks, such as cleaning, laundry, shopping menu planning, preparing and cooking meals

4. Support individuals in and out of bed, dressing, undressing, personal care and showering/washing in line with personal support plans

5. Ensure all personal care tasks are carried out the respect for privacy and dignity, and in line with the needs, wishes and preferences of the individual as detailed in their support plan

6. Promote healthy lifestyles for individuals, liaising with other professionals and agencies as appropriate

7. Undertake specific health related tasks and implement plans of support developed by members of the multi-disciplinary team following training. Administer medication and adhere to safe moving and handling processes in line with agreed policy and individual support plans.

8. Support individuals to access a range of medical appointments such as doctors, chiropodist and opticians. Implement treatment regimes as required. Support individuals admitted to hospitals.
