Job summary
We are looking for a Registered Nurse or Social Worker with significant experience in Continuing Health Care (CHC) to join us in developing our CHC service in Buckinghamshire.
We currently have a small, virtual CHC team with a part-time manager to fulfil our CHC function for Buckinghamshire. However, we are in the fortunate position of being able develop our service and are looking for an energetic and knowledgeable full time Team Manager (TM) to be able to lead the Team from virtual to a dedicated Team.
Location:The post will be based in Walton Street, Aylesbury.
Contract:This is a 12 month fixed term contract
We will consider a secondment agreement from the NHS but you will need to discuss your application with your own service and have their agreement to release you.
Main duties of the job
You will work alongside other Social Work Teams and your extensive knowledge of CHC will be essential to being able to develop and lead the Local Authority team and service to ensure the best outcomes for people using services.
Adult Social Care Team Managers are responsible for the day to day operational management of the following key areas:
- Developing a strength-based culture in practice, leadership and coaching of the team
- Compliance with all statutory Social Care requirements including the Care Act, the Mental Health Act, the Mental Capacity Act and Buckinghamshire Multi-agency Safeguarding procedures
- Contribute to service improvement to deliver outstanding support to Buckinghamshire residents
- Contributing to budget and financial management
- Performance against statutory and locally determined indicators
- Partnership working with internal and external stakeholders
You will also bring significant knowledge and skills in the CHC process with a measured approach to the understanding of the points of challenge between the Local Authority remits and position and NHS partners.
You will bring your in-depth knowledge of legislation and statutory guidance and be familiar with working to a strength-based approach where you think outside of the box and enable people to prevent, reduce or delay the need for care.
You will show resilience, persistence and a supportive and adaptable approach in working in an outcome focused way with our NHS partners.
