Main duties of the job
You will bring to the role:
- knowledge and understanding of current learning disability practice and the role of a multi-disciplinary team, including statutory and voluntary agencies
- skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities
- ability to build good working relationships with professional colleagues and partners and engage positively with individuals, families and carers
- experience of using a person-centred, holistic approach and creativity in problem solving and reflective practice techniques

As a Newly Qualified Nurse Care Manager you will be central to a network of partners working across health and social care in Leeds who are committed to increasing integration for better services supporting people with learning disabilities.
