Adult Social Care are looking to recruit one Social Care Worker to join
Hospital Social Work Team (HSWT), this is an unqualified position.
Main duties of the job
The role would require you to work within a fast-paced environment, within a supportive Hospital Social Work Team facilitating timely and safe hospital discharges. The team work to facilitate rapid hospital discharge and complete assessments post discharge in line with the Discharge to Assess Guidance. Regular supervision and training are provided. You will hold your own caseload within a supportive team of qualified and unqualified workers.

to keep people in their communities, with their families, with residential care
only used, when necessary, with the focus on strength-based practice.
We recognise that social care can be a
challenging job and actively invest in supporting our social work teams.
If
you are interested in applying for this role you will need a working knowledge
of legislation relating to Adult Social Care, Safeguarding Adults and Mental
Capacity. You will have excellent communication skills as well as skills in
assessment, partnership working and accountable practice.

Job responsibilities
The purpose of the job is to use strength and assets based
approaches to assess customers (aged 18 to older age) social care needs as
defined by The Care Act, including deciding on their eligibility for support.
The role includes developing and reviewing support plans to ensure that they are
aimed at preventing, reducing and delaying future care needs, helping customers
to manage risks in the way they choose and promoting resilience through the
development of customers abilities and those of their network of support.
The post holder will assist in identifying outcomes with
customers and carers, based on customer and carers definitions of what a good
life looks like. They will explore options for meeting these using a high level
of creativity and innovation to maximise the ability of the customer to self
care and self manage, including utilising their family and informal network of
support. The post holder will have a good knowledge of available voluntary,
statutory and health services.
In carrying out any or all of the following tasks
the post holder will be expected to pay due regard at all times to the
Councils stated policies relating to customer service and equal and fair
treatment for all customers and employees.
