An exciting opportunity has arisen to join a specialised service within Adult Social Care, where youll play a key role in making a positive impact on individuals lives and personal journeys.
Youll be part of our Outreach team within Disability Day Opportunities a community-focused service providing tailored support to adults with learning disabilities and Autism. Our staff meet individuals at home or their chosen meeting place, engaging in meaningful activities that reflect their personal interests and preferences.
As aSenior Support Worker, you'll take a lead role in shaping personalised support plans that are both person-centred and outcome-focussed. You'll also collaborate with community stakeholders to source appropriate activities and foster group interaction, promoting social inclusion and wellbeing.
Main duties of the job
- Provide day to day support and direction for the staff team.
- To act in a duty capacity for all contact regarding the daily running of the service.
- To provide enhanced support to enable individuals to fully access all Day Opportunities.
- To provide cover in the absence of the Assistant Disability Day Opportunities Managers.
- Work with vulnerable adults who have learning disabilities, helping the person to achieve personal outcomes, ambitions, and maximise opportunities, taking control of their lives and making their own choices. Enabling the person to grow relationships and build networks in their local community.
- Working creatively and proactively to ensure person centred approaches are used for people with disabilities. To work under the managers guidance to follow support plans.
- Utilise the public transport system to access the local community and public services. To lone work in the community with individuals you are supporting. Where applicable, to make use of the company pool car to transport individuals to and from specific activities.
- To maintain electronic contact records of your support and reporting to your line manage any concerns.

- Good knowledge of the Sandwell area and local public transport.
- Good communication, planning & organisation skills.
- A flexible and adaptive approach.
