Sandwell MBC and South West Birmingham NHS Trust are working in partnership to provide short-term reablement in a newly built integrated Social Care and Health Centre, which opened in December 2022. The centre is an 80 bedded reablement centre based on Harvest Road, Rowley Regis.

As a Reablement Care Assistant you will work with the service user through their person-centred plan and treatment programme in conjunction with the Therapy team and Management team.

This will be an exciting opportunity to work in a service supporting individuals to regain their skills and confidence so that they can return home.

Main duties of the job

Key duties and responsibilities include

Assisting to reduce long-term care needs by promoting independence.

Assisting in reducing re-admissions to the acute trusts.

Undertaking duties as part of a Integrated workforce.

Promoting service user well-being and independence.

Contributing to all aspects of the physical and emotional care needs of service users.

Involvement in, and promotion of, the emotional and psychological needs of service users to instil confidence by encouraging feelings of security and personal worth.

Working Hours

17.5 hours per week

You will be required to work a mixture of early, late shifts. (7am 2.30pm and 2pm 9.30pm) These will also include working weekends and bank holidays on a rota pattern.

Additional Information

Successful candidates will be required to undergo an enhanced DBS check.

Job responsibilities

Your current duties and responsibilities are:

1. Support service users in order to help them maintain or achieve independence and self-determination. Whilst this may entail acceptable risk taking, due regard must be given to service users safety.

2. Participation in the planning of person-centred care programmes and the preparation of case reviews.

3. To act as a Keyworker, implementing care plans and treatment programmes in conjunction with the Therapy Team and Management Team and record their progress, providing recommendations within the review process.

4. To assist service users in all aspects of their physical care where necessary, with special emphasis on encouraging their independence.

5. Involvement in and promotion of the emotional and psychological needs of service users to instil confidence by encouraging feelings of security and personal worth.

6. To support service users to participate in domestic tasks (e.g. cooking, laundry, care of linen and clothes, washing up and other kitchen duties, cleaning and bed making) with a view to regaining independence.

7. Ensure effective and safe use of equipment and other aids that have been assessed and provided to individuals.

8. Assist with the control of infection including the safe disposal of clinical waste, working within infection control policies and procedures.

9. As a member of a team to promote a consistent approach needed to meet the Units objectives.

10. To have regard, at all times, to the comfort, well-being and self-respect of all service users.

11. Work collaboratively with individuals, families, carers, communities, and other professionals to ensure the best outcomes are reached for individuals.

12. Ensure that all activity is accurately documented in the appropriate records for continuity of care in accordance with policies and procedures.

13. To comply with all Divisional, Departmental and Council Policies and Procedures.

14. Report any accidents/complaints/untoward incidents to the Management Team in a timely fashion and in accordance with existing policies and procedures.

15. To report any incident of ill treatment to a resident, to the Manager and, if not satisfied with appropriateness of the action taken, to Senior Management.

16. To participate in the operation of the Councils Appraisal Scheme.

17. Championing and executing an open culture of Equality, Diversity and Inclusion (EDI) throughout the council which reflects the councils policy, values and behaviours.

18. Such other duties as may be appropriate to achieve the objectives of the post to assist the Service Area in the fulfilment of its objectives commensurate with the post holder's salary grade, abilities and aptitudes.

19. The post holder must at all times carry out his/her responsibilities with due regard to the Council's policy, organisation and arrangements for Health and Safety at Work.

  • Experience of working in a care setting,
  • working to CQC standards.

Qualifications

  • NVQ/QCF/RQF L2 in Care or equivalent, or
