Main duties of the job
Work as a key member of the Local Access Point (LAP) team, delivering rapid Occupational Therapy assessments in the Slough community following referrals from the Community Hub.
Use clinical expertise and professional judgement to determine whether service users can be supported through the Integrated Care Team or other community resources, to prevent unnecessary hospital admissions.
Apply a proactive, problem-solving approach that may include providing assistive equipment, signposting to relevant services, arranging short-term rehabilitation, and reviewing or recommending care packages.
Assess service users' needs and implement short-term or safety-focused interventions to prevent deterioration and support safe, independent living at home.
Consider the impact of the service user's condition on informal carers and next of kin, ensuring a holistic, person-centred approach to care planning.
Collaborate with multi-disciplinary professionals to enable residents to achieve maximum independence within their own homes.
Where clinically appropriate, refer residents for Care of the Elderly Step-Up beds via the BHFT Bed Manager, in coordination with the Community Matron and ARC Manager at Upton Hospital.
