We are looking for enthusiastic and motivatedLevel 1 Social Workersto join our Hospital Social Work Team based at Lincoln County Hospital.Our team also supports patients across Northern Lincolnshire and Goole hospitals, including Grimsby, Scunthorpe, Hull and Goole, working closely with health colleagues to ensure safe, person-centred discharges.
This is an exciting opportunity for newly qualified or early career social workers to develop their skills in a dynamic environment while being supported by an experienced and welcoming team.

Main duties of the job
As a Level 1 Social Worker, you will:
Manage a caseload with appropriate supervision, developing your professional confidence and autonomy
Undertake Care Act 2014 assessments, care and support planning, and reviews
Work collaboratively with individuals, families, carers and professionals to identify strengths and promote independence
Contribute to safeguarding enquiries, ensuring the persons voice and desired outcomes remain central
Work closely with health colleagues to support safe and timely hospital discharges
Identify wider needs such as housing, health or community support and work with partners to address them
Promote strengths-based practice and support people to build and maintain support networks
You will be supported by experienced practitioners and leaders, with opportunities to develop your practice, confidence and career progression within Adult Care.
