Job summary
Lincolnshire County Council has an exciting opportunity in for aSocial Worker Level 1position based within the Adult Care Team at Pilgrim Hospital, Boston. This role would be suitable for a newly qualified Social Worker requiring their ASYE or a Level 1 candidate looking for more experience in a hospital setting.
You will be undertaking assessments of need both within hospitals and the community settings to ensure our customers are able to live independent lives and for those that need support, the right support package, in the right place, at the right time. The customer is at the centre of the process; our highest continuing aim is to provide the best service possible to service users and their families. The aim of the team is to work hand in hand with NHS and third sector staff to ensure that people we support are at the very centre of their own discharge. Working creatively when planning support packages, thinking outside of the box to ensure an individual care plan is in place that empowers the service user to participate.
LCC is leading the way on the introduction of a fully integrated Discharge to Assess Hub, to support access to the right services, reducing delays and delivering benefits.
Work is undertaken face to face, on site, to continually offer the best service we can to those people who we support.
You will work as part of the 7-day Adult Care rota. This equates to approximately 1 in 4 weekends worked, and these are all scheduled 3 months in advance.

Responsible for providing statutory professional social work as part of a team supporting adults
with care and support needs. You will demonstrate a commitment to strengths based practice
and skills in supporting people to identify and build support networks to achieve their identified
outcomes. You will be responsible for assessment, care and support planning, implementing
and reviewing packages of care and support. You will provide advice and support to other Adult
Care practitioners in relation to their cases. You will be expected to work with autonomy
managing a case load and demonstrate a multi-agency approach integrating your practice with
colleagues across the health and care sector. Work may include travel outside the county.
MAIN DUTIES:
Manage a caseload with supervision and advice from the Lead Practitioner at a level ofcomplexity appropriate to the post holder's stage of professional development andcapability and be accountable for their professional practice.
Undertake needs assessments in line with the Care Act 2014 to identify care and supportneeds adopting and promoting a strengths based approach to identifying, outcomespeople want to achieve, opportunities for independence and identify solutions
To work with specialist safeguarding colleagues to contribute to and undertakesafeguarding enquiries which put the person and the outcomes they want to achieve at thecentre of safeguarding practice in line with LCC safeguarding policies and procedures.
To identify non eligible and non-social care needs such as health care or housing and offerinformation and advice and work with partners to address wider needs.
Construct value for money care packages in collaboration with individuals, carers andcontributing agencies in accordance with eligibility criteria and ensuring that risk isminimised and quality of life improved/maintained.
Work with individuals, families, carers and communities to help them make informed choices and decisions, enabling them to clarify and express their needs and contribute to service planning.
Meet identified needs through informal support and/or through resources and assets available in the public, private and voluntary sectors.
Research resources and services available for people in need or at risk, identifying and reporting any gaps in service provision.
Operate within the delegated budget support mechanisms as they are developed.
Maintain and update cases notes and other records, write reports as required; if required, give evidence in court in relation to care proceedings.
