About the Role

You will have an allocated case load which includes complex cases.Working in a Person-Centred way, carrying out strength based Adult Needs Assessments and Reviews focusing on what the person can do rather than what they cannot do. Working with the people wesupportand their families to maximise independence.

Our Level 2 qualified practitioners,support the wider team, by completing strength-based supervision, audits and appraisals with nonqualified team member, tosupport and develop their practice and quality.

Our aim is tosupportpeople to reach their goals and aspirations. Whether that be employment, community access or moving into their own home. We help people to overcome the obstacles and challenges they may face,supportingthem to reach their potential.

You must have use of a vehicle, be able to travel effectively and within a reasonable timescale to meet service requirements, this could include beingresponsiveto last minute unplanned visits which may be non local/out of county appointments.

As well assupporting our customers you will supervise up to three non-qualified team members,supportingtheir development and professional practices. Completing observation of practice, Audits and casesupport.

Job responsibilities

PURPOSE OF JOB

Responsible for providing statutory Adult Care and Health support as part of a multi-disciplinary team supporting people with learning disabilities with care and support needs.

You will demonstrate through practice experience a commitment to strengths-based practice and supporting people to identify and build support networks to achieve their identified outcomes.

You will be responsible for assessment, care and support planning, implementing and reviewing packages of care and support on a varied and more complex case load.

You will be expected to work with autonomy managing more complex work and demonstrate a multi-agency approach integrating your practice with colleagues across the health and care sector. You will provide advice, support and mentoring to other Adult Care practitioners in relation to their cases. Work may include travel outside the county.

MAIN DUTIES:

1.Manage an allocated caseload of clients, regularly monitor, review and evaluate changes in clients' needs and ensure that all services contribute effectively to the care plan and it remains an effective use of resources.

2.Undertake a range of community care and safeguarding assessments and reviews in accordance with relevant statutory requirements, identifying risks, needs and options, including self-directed support options.

3.Supervise up to four non-registered social care practitioners e.g. Practitioner (Unqualified), including planning, allocation and checking of work including setting targets, dealing with day-to-day capability and conduct issues / providing basic staff welfare and support/ identify training needs.

4.Construct value for money care packages in collaboration with users, carers and contributing agencies in coordination with other professionals, families and individuals, and in accordance with eligibility criteria and ensuring that risk is minimised and quality of life improved/ maintained.

5.Work with individuals, families, carers and communities to help them make informed choices and decisions, enabling them to clarify and express their needs and contribute to service planning.

6.Maintain and update case notes and other records, write reports as required, give evidence in court in relation to care or other proceedings.

7.Convene and chair multidisciplinary planning and review meetings for individuals with complex needs to ensure a coordinated response to service delivery.

8.Identify, track and assess young people in the transition into adulthood as specified in the transition protocol.

9.Promote health in people with learning disabilities and support them to access healthcare.

10.Provide advice and guidance to individuals and families on service and support providers and options including signposting to other agencies. Communicate with individuals and their families in ways which are appropriate to their needs.

11.To undertake supervision of students and participate in training and staff development programs.

12.Ensure the promotion and completion of carers assessments.

PERSON SPECIFICATION:

Professional qualification as a Social Worker Or Registered Nurse e.g. RNLD, RGN, RNMH.

Current HCPC Social Work or NMC Nursing Registration.

Minimum of 2 years post qualification practice experience in a health and social care environment.

Knowledge of legislation relevant to provision of health and social care for people with learning disabilities including the requirements of the health and safety legislation and Valuing People.

Knowledge of health and health promotion issues specific to people with learning disability.

Knowledge of screening procedures and how they are used to effectively target resources.

Interpersonal, verbal and written communication skills.
