About the Role
You will have an allocated case load which includes complex cases.Working in a Person-Centred way, carrying out strength based Adult Needs Assessments and Reviews focusing on what the person can do rather than what they cannot do. Working with the people wesupportand their families to maximise independence.
Our Level 2 qualified practitioners,support the wider team, by completing strength-based supervision, audits and appraisals with nonqualified team member, tosupport and develop their practice and quality.
Our aim is tosupportpeople to reach their goals and aspirations. Whether that be employment, community access or moving into their own home. We help people to overcome the obstacles and challenges they may face,supportingthem to reach their potential.
You must have use of a vehicle, be able to travel effectively and within a reasonable timescale to meet service requirements, this could include beingresponsiveto last minute unplanned visits which may be non local/out of county appointments.
As well assupporting our customers you will supervise up to three non-qualified team members,supportingtheir development and professional practices. Completing observation of practice, Audits and casesupport.

The ideal candidate will need to hold either aSocial WorkorNursequalification and be able to show that their registration with SWE/NMC is in date. The nurse qualification does not need to be specific to Learning Disability.
