We are delighted to offer this recruitment opportunity for aQualified Practitionerposition within the Preparation for Adulthood Team. The post is full time and a permanent position.

The Qualified Practitioner post is both an exciting and challenging role. For those who love fast paced and 'make a difference' work, this is the position for you. You will need to hold either a Social Work or Nurse qualification and be able to show your registration with SWE/NMC is in date. The nurse qualification does not need to be specific to Learning Disability.

Main duties of the job

The Preparation for Adulthood Team operates county wide within Adult Care and Community Wellbeing and their main priority is to support effective transition for eligible young people moving from Children's to Adult Services.

The teams priority is supporting young people to make informed choices about their future and assisting them to achieve the best outcomes during transition and as they enter adulthood.

Working in partnership with Children's Services and relevant care and health partners, each staff member works alongside relevant local schools, attending Education health and care (EHC) reviews and providing information and support to young people, their families/carers and to our colleagues in educational establishments.
