We are specifically looking for a Support Worker to join two of our teams: our dedicated Social Care Support Team who undertake this essential work within the prison setting at HMP Littlehey and our Enhanced Response Service team based in Cambourne. Our Prevention and Early Intervention (P&EI) Services work hard to deliver multi-disciplinary social care interventions to prevent, reduce and/or delay escalations in formal or informal care and support needs. Within P&EI, our Support Workers deliver planned and personalised short-term care and support. They enable people to recover and maximise function, confidence, and independence. Support Workers within the prison setting work in pairs, with no lone working and will be provided will full training and support. ERS is a fast paced service providing support to service users who trigger their Tele Care Alarm and we also work alongside the ambulance service for non injuries such as non-injured falls. ERS also provides support to teams within the Cambridgeshire County Council such as the Reablment and Emergency Duty teams to provide planned calls to service users in the community to support with promoting independence. This is a permanent position, totalling 35.25 hours per week. 21 hours per week will be spent at HMP Littlehey and 14.25 hours per week at Cambourne for which mileage expenses will be paid. The teams operate across 7 days a week, 365 days a year, so you will be required to work some evening shifts, early shifts and every other weekend. Additional and enhanced rates for weekend and Bank Holiday working will apply. Occasional travel across the county for meetings will be required. Within the role at Littlehey, you will be working closely with health and social care professionals who are involved in the planning and delivery of Reablement care and support You will be supporting the development of person-centred planning, where each persons plan has meaningful and achievable goals and sssisting with activities of daily living and personal care, including washing, dressing/undressing, bathing and meal preparation. Your role will centre on helping people regain independence by motivating them to re-learn daily skills and supporting ongoing care need provisions Our ERS Support Workers also assist with the provision of all personal care such as washing, dressing, toileting, catheter care, feeding, meal preparation, medication and others. We respond to falls and telecare alerts to provide reassurance and practical assistance in co-ordination with other services, such as Fire and Rescue and Ambulance. As part of the team you will apply learning from adult protection, diversity and mental capacity act training, you will be responsible for recognising and reporting any situations, incidents or concerns in line with Council Policy. You will be required to liaise effectively within the team and with other agencies e.g. assistive technology or health colleagues, to ensure the service is delivered effectively. You will also be required to complete both written and electronic documentation regarding our service users care in accordance with CCC policy. Main duties of the job
Our Social Care Support Workers are motivated people of action. You will have the ability to demonstrate reliability, dedication and strength of character. You will have first-rate communication and listening skills, excellent interpersonal skills, with an element of patience and a tactful approach. You will be organised and self-motivated in order to manage your workload and will have a flexible approach to your working pattern. You will have a level 2 Care certificate or equivalent qualification and be willing to work towards a level 3. The role involves travel from Littlehey to Cambourne and also across Cambridgeshire, using CCC pool vehicles which are manual drive so you will need to have a full driving licence applicable to manual vehicles.
