Job description

Job responsibilities

This is an exciting opportunity to join Cambridgeshire County Council as an Adult Community Support Coordinator in our Huntingdon and East Cambs (Ely) Adult Community Team.

The role comes with responsibility for completing Care Act assessments and reviews as well as support planning, completing Mental Capacity Assessments and carrying out Safeguarding Adult Enquiries on behalf of Cambridgeshire County Council. The role involves working with people to identify their strengths and how these can be used to support them in meeting their assessed needs. This is all in line with the Care Act 2014, as part of strengths-based and person-centred interventions. The post holder will work with vulnerable adults with a range of social care requirements including disabilities.

What will you be doing?

You will be expected to work in partnership with service users, carers and other professionals within and outside of the council. This will include undertaking duty on a rota basis, visiting people to carry out face-to-face assessments and reviews, and carrying out reviews and assessments over the phone. You will need to show dedication to establishing needs and ensuring that they are met, to empower our service users and facilitate their independence.

You will be carrying out the role of Safeguarding Officer, and reporting to managers during safeguarding enquiries, as well as keeping records accurate and up to date.

About you

You will need to understand Adult Social Care legislation including the Care Act 2014, Safeguarding Adults and Making Safeguarding Personal principles as well as the Mental Capacity Act 2005.

You will be expected to work collaboratively as part of a team and demonstrate commitment to making a difference and building relationships.

You must have the ability to travel to a variety of locations independently.
