An exciting opportunity has arisen for an Adult Support Co-ordinatorin the Young Adults Team, on a full time basis, 37 hours a week.
The Young Adults Team is a countywide team based at Scott House. TheYoung Adults Team support Adults 18-25yrs with a diagnosed learningdisability who have an EHC Plan. We are looking for a dynamic,committed and passionate individual who is dedicated to improving the livesof young people, working alongside other professional colleagues to promotethe principles of personalisation, encouraging independence, choice andopportunities for young people.
There is some part of the role which involves working alongside Childrensservices with young people under 18 years to prepare them and their familiesfor adulthood. Candidates will have the ability to support service usersthrough change, encouraging progression, and acknowledging individual,family and community strengths through a transforming lives approach.
Occasionally there may be some out of county travel required.

What will you be doing?You will be one of 4 Adult Support Coordinators , working along side Social Workers, Senior Social Workers and closely with LDP health colleagues.
You will hold a caseload of young people between 16 and 25 and will need tocomplete adult social care functions such as assessments, support plans,reviews, MCAs. You will also need to cover the teams duty function a coupleof times per month on a rota basis. You will work closely with education,health, social care providers, children's and adults' social care teams.
Adult Support Coordinators also complete statutory visits to some of ourYoung people who are defined as a care leaver. You will have access to teamhuddles to discuss your cases and seek support from colleagues twice a week.
There are opportunities to represent the team by attending engagementevents in the community, speaking with young people and theirfamilies/carers. You will receive monthly 1:1 support from your line managerand will have access to a duty Senior Social Worker for support and advice atall other times.
About youKnowledge and experience of working with people with a learning disability isdesirable but not essential. Working knowledge and experience of the CareAct, Mental Capacity Act and CCC Adult Social Care policies and procedureswill also be an advantage.
You need good written and verbal skills, the ability to work at a timely pace, good organisational skills that will enable you to manage a caseload and ability to provide good service to our young people and families.
You need to take a creative, strengths based approach and be solution focused,while keeping the young adult at the centre of the process.
You will need to work as part of an MDT at times and be able to organise andChair meetings.
You must have the ability to travel to places that public transport does not support both within, and occasionally outside of, Cambridgeshire.
You need to be able to work at the office for a minimum of 2 days per week andwork from home on other days.
