Job summary

We are seeking an experienced Specialist Community Public Health Nurse SCPHN Specialist Practitioner to play a pivotal role in delivering the Healthy Child Programme and improving health outcomes for vulnerable children, young people and families. In this position, you will undertake comprehensive assessments and lead the management of Child Protection and Child in Need cases. You will offer specialist clinical expertise, ensuring the highest standards of evidence based care while championing a child centred approach across all elements of practice.
As a SCPHN Specialist Practitioner within the 019 Public Health Nursing Service, you will provide professional supervision, guidance and support to practitioners, helping to build capability and consistency across the workforce. You will work collaboratively with multiagency partners, contribute to clinical governance, prepare high quality reports, and support the development and implementation of policies and pathways. This is an excellent opportunity for a motivated SCPHN who is passionate about reducing inequalities, influencing practice, and driving positive outcomes for the most vulnerable families.
Main duties of the job
To provide evidence based public health nursing practice that promotes and improves health and addresses inequalities of the most vulnerable and disadvantaged children, young people and families within the borough. Lead the delivery of the healthy child programme to improve health and wellbeing of children in need. Provide Specialist Community Public Health Nurse (SCPHN) expertise to develop and implement structured interventions for children and families with complex needs including assessments, appropriate management of risks and providing supervision to other team members.
