About the post
We are looking for an experienced SCPHN Health Visitor or School Nurse with substantial understanding of the complexities of working with vulnerable families and the impact of abuse and neglect. The postholder will provide evidence based public health nursing practice that promotes and improves health and addresses inequalities of the most vulnerable and disadvantaged children, young people, and families within the borough. You will lead the delivery of the healthy child programme to improve health and wellbeing of children in need, providing SCPHN expertise to develop and implement structured interventions for children and families with complex needs who are subject to safeguarding plans. The postholder will attend strategy meetings, Initial case conferences, review conferences, core group and child in need meetings, delegating as necessary workload to skill mix members within teams as appropriate. The role will lead, support, and strengthen safeguarding knowledge and skills within the wider service.

To provide expert clinical leadership within the 0-19 Public Health Nursing Service (PHNS) in delivery of the Healthy Child Programme, working in partnership with key stakeholders, to ensure better outcomes for more vulnerable families within the Borough Undertake clear and detailed assessments, effectively identifying, risk assessing and analysing the needs of more vulnerable children, young people and families in accordance with policies and procedures. To provide specialist knowledge, supervision, advice and support to PHNS staff working with communities, families, individuals with vulnerability, complexity and safeguarding needs and where individual case or caseload management support is required Leading on and effectively managing complex situations, assessing, and managing high levels of risk ensuring step up and step down is effective in continuity of care within the service.
