OurAssistant Care Managers have a really supportive working environment and inductions are well planned with help from across the team. There are 25 members in theWorthing Hospital Social Work Team so whether it's in person, on the phone or even in the chat group there is always someone on hand to help.
You will be working across the Acute ward, A&E, Community Hospitals, and assessing people in their own homes; providing advice and guidance to those people who access or need the service.. As well asassessing you will be completing plans, implementing and monitoring care packages, although there is a focus on short term support you will also be carrying out reviews for long term support.To be successful in this role you will need to use your initiative and demonstrate personal resilienceas you could have exposure to challenging behaviours from people who access or need the service.
An important part of the role isaccurate records of all of the amazing work you have undertaken, and produce reports and correspondence for a variety of audiences including meetings, conferences, court, panels and other agencies.

In this exciting opportunity, you will, under guidance and regular supervision provide a social work support service to those people who access or need the service, to include assessment, planning and review within the Supporting Lives, Connecting People framework. You will manage a challenging and complex portfolio of casework, attending multi-disciplinary meetings and working in partnership with our Health, Community and Third Sector colleagues.

There is a requirement to have good up-to-date working knowledge of legislations relevant to the specialist area of the post. Demonstration of good resilience is required as you could have exposure to challenging behaviours from people who access or need the service.
