The aim of Adult Care Point 2 - Initial Contact (ACP2) is to meet need at the earliest opportunity in a persons journey. As part of a multi-disciplinary team that supports, prevents, reduces and delays the need for long term services, you will provide specialist advice to social care professionals and social workers within the ACP2 team. You will work as part of a strong and dynamic Occupational Therapy service, undertaking strengths-based assessments (as directed by the Care Act 2014) utilising Occupational Therapy skills and knowledge to develop and implement a plan of support, aimed at improving wellbeing and delivering outcomes for people. You will be required to undertake a duty function, completing telephone assessments of needs with customers approaching social care for advice, information, equipment and adaptation (e.g. stair lifts and level access showers). You'll be expected to use up to date evidence-based practice to inform your intervention.
Physically, you will need to be able to sit for prolonged periods of time during the telephone assessments using a head set and or iPhone.
Main duties of the job
Main duties of the role include the below (please refer to the job description for full list):
- Undertake strengths-based assessments and reviews; gather information, record accurately, and make decisions to support the persons needs and outcomes.
- Proactively work with customers, carers, colleagues, and other partners to achieve outcomes. Monitor and review arrangements as appropriate.
- Maintain key working relationships with internal customers, team colleagues and external stakeholders including housing authorities, in order to meet housing needs including making recommendations for home adaptations.
- Implement Occupational Therapy interventions, including prescribing assistive equipment and technology, designing adaptations to peoples homes, and providing information and advice to prevent, reduce or delay the need for ongoing support.
