Job summary
Warwickshire County Council is pleased to announce that we are looking for Occupational Therapists to join Warwickshires Reablement team.
Due to expansion of the service we have OT posts in reablement. We are a well-established team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, ReablementSupervisors, Reablement Assistants and Social Care Practitioners, who have provided years of interventions for Warwickshire Reablement customers and aim to achieve the best outcomes going through their Reablement journey.
Reablement focuses on promoting independence by supporting customers to do as much as possible for themselves both physically and psychologically. We do this by supporting customers to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home.
We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and well-being.
Main duties of the job
Reablement Occupational Therapists complete comprehensive assessments with customers who are struggling to manage activities of daily living in their homes or who have returned home from hospital following a period of ill health. Reablement customers have the opportunity to work on outcomes that are important to them.
You will be working within the service to respond preventatively to referrals for Social Care and support and will use Occupational Therapy interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence, and to enable / support customers to achieve their desired outcomes.
You will provide professional support and guidance to the team, using knowledge and expertise for complex cases. You will receive professional support from a Senior Occupational Therapist.
Having good time management/organisation skills is required; being solutions focussed flexible in your approach, willing to travel across the county as needed, as well as having experience of handling confidential information with tact and discretion.
The post requires you to work a shift pattern of 9 am - 5 pm and 12 pm - 8 pm Mondays to Fridays on a rota basis, including one Saturday in every 7 weeks. There will also be requirement to work bank holidays on a rota basis.
